The Role: Community Registered Nurse

We’re looking for experienced Registered Nurses, Band 6, to join our Virtual Care Team at HomeLink Healthcare. You’ll play a key role in assessing, monitoring, and escalating patient care remotely, helping people stay safely at home and avoid unnecessary hospital admissions.

You’ll use your clinical judgment and decision-making skills to identify deteriorating patients, interpret observations (including NEWS2), and escalate concerns to clinicians or emergency services when required. You’ll communicate with patients, carers, and the wider team via phone, video, and digital systems, ensuring safe, high-quality, and compassionate virtual care.

About You

You’re a confident, NMC-registered nurse with strong IT and communication skills and proven experience recognising and escalating deteriorating patients. You’ll be comfortable working autonomously and using digital monitoring systems.

You’ll be responsible for contributing to the holistic assessment, planning, delivery and evaluation of patient care.

Essential:

  • Registered Nurse (NMC)
  • Band 6 experience
  • Confident in using NEWS2 and clinical escalation
  • Strong IT and remote monitoring skills

Desirable:

  • Experience in community, virtual ward, or telehealth settings
  • Leadership or supervisory experience
